You have a new, isolated medical problem that needs evaluation and treatment, and you do NOT need a comprehensive health review or chronic disease management.

What's included:

  • Focused evaluation of your acute complaint.

  • Clinical assessment and diagnosis.

  • Treatment recommendations (e.g., prescription, home care instructions, or referral if needed).

  • Brief counseling on when to seek emergency care.

Common acute issues we treat:

  • Urinary tract infections (UTI).

  • Sinus infections or upper respiratory infections.

  • Sore throat or ear infections.

  • Skin rashes or minor wound care.

  • Cough or cold symptoms.

  • Nausea or GI upset.

  • Medication refill for patients on stable regimens.

Important note:
If during this visit we discover you need chronic disease management, medication review, or a more comprehensive evaluation, we may recommend converting to a Comprehensive Intake visit. A $55 fee difference would apply.
