Appointments can be scheduled for up to 6 weeks out.

You are new to our practice and seeking to establish care as your primary healthcare provider, or you have complex medical needs requiring a thorough evaluation.
15 minutes
When to choose this:
You have a new, isolated medical problem that needs evaluation and treatment, and you do NOT need a comprehensive health review or chronic disease management.
When to choose this:
Includes Initial comprehensive intake + 6 acute/urgent visits.
For when you want to establish care with us and have the peace of mind of prepaid access to acute care throughout the year.
